A parent consent form is required when a minor will participate in some activities alone or with a group. Going through medical treatment or going on a field trip with the school are good examples of situations where parental consent is required.

Generally, under North Carolina law, the consent of a parent, guardian, or person standing in loco parentis is required to provide health services to a minor, which is a person who is less than 18 years of age. There are exceptions to this requirement, one of which is set out in North Carolina's minor's consent law.

Who may make an authorization to consent to health care for minor. Any custodial parent having understanding and capacity to make and communicate health care decisions who is 18 years of age or older or who is emancipated may make an authorization to consent to health care for the parent's minor child.
