Know Your Responsibilities: All five states except Georgia require two witnesses. In certain states you can act as both Notary and witness, while in others you can't. In Connecticut, Florida and South Carolina, the Notary may act as a witness; in Georgia, Louisiana and Michigan the Notary may not.

Be specific. Your acknowledgment receipt should contain the names of the issuing party and the person receiving the document. A description, with the name of each document being issued, date of the issue and purpose of the document should be clear.
