(1) 'Advance directive for health care' means a written document voluntarily executed by a declarant in ance with the requirements of Code Section 31- 32-5. (2) 'Attending physician' means the physician who has primary responsibility at the time of reference for the treatment and care of the declarant.

Almost all states permit advance directives for healthcare, which can be used to direct at least some forms of psychiatric treatment. In the past decade, twenty-five states have adopted specific psychiatric advance directives statutes.

Alaska Advance Health Care Directive An advance health care directive allows you to name someone else to make health care decisions for you if you are unable to make your wishes known, as well as allowing you to give instructions in advance about some treatments.

Advance directives consist of (1) a living will and (2) a medical (healthcare) power of attorney. A living will describes your wishes regarding medical care. With a medical power of attorney you can appoint a person to make healthcare decisions for you in case you are unable to speak for yourself.

Under a durable power of attorney, the health care agent may refuse or consent to medical care or pain relief, but may not authorize the removal of life-sustaining care unless specifically indicated in the patient's living will. A durable power of attorney is revocable at any time in Alaska.
