A deed must meet the following requirements to be valid: (1) be in writing; (2) name the parties; (3) give the legal description of the property; (4) specify the legal rights being conveyed; (5) include a consideration; and (6) be signed by a competent grantor and witnessed by two persons.

Form of quitclaim deed -- Effect. A quitclaim deed when executed as required by law shall have the effect of a conveyance of all right, title, interest, and estate of the grantor in and to the premises therein described and all rights, privileges, and appurtenances thereunto belonging, at the date of the conveyance."

The completed form will need to be submitted to the Utah County Recorder of the same county as that of the property.
