A Durable Power of Attorney for Health Care is a document that lets you name someone else to make decisions about your health care in case you are not able to make those decisions yourself. It gives that person (called your agent) instructions about the kinds of medical treatment you want.

A Missouri advance directive is a document that outlines an individual's preferred medical treatment options and an agent to speak on their behalf in the event the person can no longer speak for themselves. In case of such an event, the agent will be the sole decision-maker in all health care matters.

Advance directives are written statements prepared ahead of time that allow caregivers to know of a patient's wishes for treatment or that legally specify an alternate decision maker should the patient lose decision-making capacity.
