Service of process by mail shall be made by registered or certified mail, return receipt requested, and shall be complete upon the return of the receipt signed by the defendant to the court. The signature of the defendant on the return receipt shall constitute prima facie proof of service by mail.

A $166 filing fee will be due at the time of filing. If you are unable to afford the filing fee, you can request an exemption by filing a Motion and Affidavit for Fee Waiver (FW 1-9) along with an Order Re: Fee Waiver (FW 1-10).

You will need file with a certified marriage certificate and your current driver's license or your U.S. passport. No processing fee will be charged. Roughly two weeks after your form is processed, you will receive a new SS card in the mail. The card will show your new married name, but your SSN will stay the same.
