This document says three things legally: That the person was employed by the employer who issued the Certificate; That the employee was performing a specific function within the business; and. The employee performed these functions within this time period.

It is normally the responsibility of line managers to keep in regular contact with any of their staff who are absent. They are usually best placed to know the individual and to be able to discuss any sensitive issues. If it's a minor illness that is likely to end within 5 days then contact is not usually necessary.

A sick note or certificate of incapacity to work (Arbeitsunfahigkeitsbescheinigung/AU-Bescheinigung/Krankschreibung/gelber Schein) is the certificate you get from your doctor when he/she determines that you cannot work because of illness. It usually states the day on which you saw the doctor.

Provide as much notice as possible that you won't be attending work. If you can tell your boss that you will be taking a sick day the night before, or even as you are leaving the office, then do so. Otherwise, call, email or text first thing in the morning.

Fill in the debtor's name and mailing address. It may be an individual, or it may be in the name of a business or organization. If the loan is in the name of the business, include the business mailing address. There is space for additional debtors. Include them exactly as they appeared on the loan agreement.
