This form is used by a Landlord to provide a residential tenant with 5 days Notice to remedy a breach in the lease agreement, in accordance with the laws of your state. (Note: If a written agreement provides for a longer notice, use the notice length stated in the agreement). "Residential" includes a house, apartment or condo. The Tenant is informed that the Tenant must either remedy the breach or suffer possible termination. If he does not remedy the breach within the 5 day period, Landlord may begin eviction proceedings.
