You may notarize a document that does not have pre-printed notarial wording as long as the signer tells you what type of notarization is needed. Signing a document filed with the court or presented to a judge or judicial officer constitutes verification upon oath or affirmation.If an individual is physically unable to sign a record, the individual's signature may be obtained in the manner provided in section 645.44, subdivision 14. If you are asked to notarize such a document, it is OK to proceed without filling in the blanks in a section reserved for official use. The signer must sign it in the presence of the notary.
