The Alaska Petition for Order Authorizing Disposition of Unclaimed Body, also known as Form 9P, is used by individuals to petition the court for an order authorizing the disposition of a body that has gone unclaimed. This form is used when the deceased individual has not known next of kin or other representative that can take custody of the body. The petition must be filed in the court of jurisdiction for the county in which the death occurred. There are two types of Alaska Petition for Order Authorizing Disposition of Unclaimed Body: 1. Petition for Burial at Public Expense — This petition is used when the petitioner is requesting that the body be buried at public expense. 2. Petition for Cremation at Public Expense — This petition is used when the petitioner is requesting that the body be cremated at public expense. The petition must be filed with the court and must include the following information: name and address of the petitioner; name, date of birth, and place of death of the deceased; name and address of the funeral home or other facility that currently holds custody of the body; a statement that the deceased has not known next of kin or other representative to take custody of the body; and a statement of the petitioner’s request for the disposition of the body. The petition must also include any supporting documentation, such as a copy of the death certificate.
