The Alabama WC Assessment Form (Instructions) is used by employers in the state of Alabama to evaluate the medical condition of an employee who has suffered a workplace injury or illness. The form is used to assess the employee's medical condition and make appropriate recommendations for treatment and rehabilitation. The form consists of three sections: Reexamination, Post-Examination, and Authorization for Treatment. Pre-Examination questions include the employee's personal information, medical history, employment history, and other relevant information. Post-Examination questions include information about the employee's current medical condition, any medication the employee is taking, details about any restrictions on the employee's work activities, and any recommended courses of treatment. Finally, the Authorization for Treatment section requires the employee's signature and the signature of the employer to authorize and approve any recommended courses of treatment. There are two types of Alabama WC Assessment Form (Instructions): the Standard Form and the Special Form.
