Arizona Proof of Claim, Attachment A is a form used by creditors to claim any money they are owed from a debtor in a court-approved bankruptcy case. The form is composed of two parts, a Declaration of Amounts Owed and a Statement of Claim. The Declaration of Amounts Owed requires creditors to provide details such as the amount of the claim, the date the debt was incurred, and any security interest that may exist. The Statement of Claim requires creditors to provide information about the debtor’s assets, including the identity of the creditor, the type of debt, and the amount owed. There are two types of Arizona Proof of Claim, Attachment A. The first is for secured creditors, and the second is for unsecured creditors. Secured creditors are creditors that have a security interest in the debtor’s assets, and they must provide additional information regarding the asset that is securing the debt. Unsecured creditors are creditors that do not have a security interest in the debtor’s assets, and they must provide information regarding the type of debt and the amount owed.
