The Arizona Employer Information Sheet is a document used by employers in the state of Arizona to notify the state of changes in their business information. It is required to be completed and submitted to the Arizona Department of Economic Security (DES). The form must be completed whenever an employer changes their: business name, business address, business phone number, or federal employer identification number (EIN). There are two types of Arizona Employer Information Sheet: the online version (Form UIA 1028) and the paper version (Form UIA 1028A). The online version is available on the DES website and can be filled out and submitted electronically. The paper version must be printed, filled out, and mailed to the DES. Both types of Arizona Employer Information Sheet must be completed and submitted in order for the state to approve any changes in an employer’s business information.
