Hawaii Partnership Change of Name Statement (Domestic Only) is a document used to legally change the name of a domestic partnership business registered in the state of Hawaii. This document must be signed by all partners of the business and notarized. The document must include the current name of the partnership, the new name of the partnership, the state of formation of the partnership, and the signature of all partners. There are two types of Hawaii Partnership Change of Name Statements (Domestic Only): Type A and Type B. Type A is used when the partnership has never filed a Certificate of Amendment of the Partnership Name with the Department of Commerce and Consumer Affairs and the partnership's name has not been changed since its registration. Type B is used when the partnership has previously filed a Certificate of Amendment of the Partnership Name with the Department of Commerce and Consumer Affairs and the partnership's name has been changed since its registration. Both documents must be filed with the Department of Commerce and Consumer Affairs in order to legally change the name of the partnership.
