A Hawaii Notice of Appeal (From Judgment Order Or Decree of Court) (Sample and Form) is a legal document used to initiate an appeal to a higher court from a judgment order or decree of court issued by a lower court. There are two types of Hawaii Notice of Appeal (From Judgment Order Or Decree of Court) (Sample and Form): a Notice of Appeal from a Judgment Order or Decree of Court and a Notice of Appeal from a Final Decree or Order of Court. Both documents must be completed and filed with the appellate court. The Notice of Appeal from a Judgment Order or Decree of Court must include the name of the lower court, the names of the parties to the case, the date of the judgment order or decree of court, the name of the court to which the appeal is being taken, the name of the appellant, and the designation of the order or judgment being appealed. The Notice of Appeal from a Final Decree or Order of Court must include all the same information as the Notice of Appeal from a Judgment Order or Decree of Court, plus the name of the court that entered the final decree or order, the date of the final decree or order, and a statement specifying the specific issues to be appealed.
