The Iowa Nonelection of Coverage Form is a document that employers in Iowa must provide to their employees to inform them of their rights to opt out of health insurance coverage. Employees can use this form to decline coverage offered by their employers and receive no tax penalty when filing their taxes. There are two types of Iowa Nonelection of Coverage Forms: the group health insurance plan form and the individual market form. The group health insurance plan form is used by employers who offer group health insurance plans to their employees. The individual market form is used by employers who do not offer group health plans. The form includes an explanation of the employee’s rights, a section for the employee to provide their name, address, and date of birth, and a section for the employee to sign and date the form. The completed form must be returned to the employer within 30 days of receipt.
