The Illinois Uniform Order For Support (IFSA) is a statewide form used by the courts in the state of Illinois to establish or modify child support and maintenance (formerly known as alimony) obligations for parents. This form is used when parents are not part of a dissolution of marriage or civil union process, and when one or both parents live in Illinois. The IFSA form is created by the Illinois Department of Healthcare & Family Services, and is used to calculate the amount of support to be paid by each parent. The Illinois IFSA form has two types: the Order Establishing Support and the Order Modifying Support. The Order Establishing Support is used to set a support amount when there is no existing court order for support in place. The Order Modifying Support is used to modify an existing court order. The IFSA form also includes information on health care costs, income withholding, and health insurance. The form requires each party to provide financial information, such as income, assets, and expenses. The form is then used to calculate the amount of support to be paid by each parent.
