The Louisiana Request For Independent Medical Examination (RIME) is a form used for the purpose of obtaining an independent medical evaluation from a qualified doctor. The RIME form must be completed by the requesting party and must include detailed information about the injured person, including the name, address, date of birth, Social Security number, and a description of the injury. The RIME form is used to request an independent medical examination (IME) from a doctor qualified to provide an opinion on the injury or illness. The IME is typically conducted by a doctor who is not employed by the requesting party and has no vested interest in the outcome of the examination. The doctor chosen for the IME must be licensed and have specific qualifications for the type of examination requested. There are two types of Louisiana Request For Independent Medical Examination: Qualified Medical Evaluator (ME) and Agreed Medical Evaluator (AME). I am typically used when a claimant is seeking a more in-depth evaluation of an injury or illness, while AME is often used when a claimant is seeking a more limited evaluation.
