A Massachusetts Military Affidavit is a document used by service members of the Massachusetts National Guard and Reserve to claim military service related benefits. It is usually used to prove eligibility for veteran's benefits, to waive military service related fees, or to obtain a fee exemption or waiver. There are three types of Massachusetts Military Affidavit: the Regular Massachusetts Military Affidavit, the Military Service Exemption Affidavit, and the Waiver of Fees or Charges (MMC) Affidavit. The Regular Massachusetts Military Affidavit is used to prove military service, and must be signed by a Notary Public. The Military Service Exemption Affidavit is used to waive certain military service related fees, and must be signed by a Notary Public and a commanding officer. The Waiver of Fees or Charges (MMC) Affidavit is used to obtain a fee exemption or waiver, and must be signed by a Notary Public and a commanding officer.
