The Maryland Americans With Disabilities Act (MD ADA) Grievance Form is a document used to report discrimination based on disability. This form is used to submit a complaint to the MD ADA Compliance Office, and is available on the Maryland Department of Disabilities website. It is available in both PDF and Word format. The MD ADA Grievance Form is divided into two parts. Part I is a “Statement of Complaint,” which includes questions about the nature of the discrimination and any relevant documentation. Part II is the “Grievance Procedures” section, which outlines the process of filing a complaint and the rights of the complainant. The MD ADA Grievance Form has three types: the MD ADA Complaint Form, the MD ADA Grievance Form for Direct Services, and the MD ADA Grievance Form for Employment. The MD ADA Complaint Form is used to report discrimination that occurs in public places, such as restaurants, stores, and other public areas. The MD ADA Grievance Form for Direct Services is used to report discrimination that occurs in the provision of direct services, such as health care, education, and housing. The MD ADA Grievance Form for Employment is used to report employment discrimination. Each type of MD ADA Grievance Form requires specific information and documentation to be submitted in order for a complaint to be processed. All forms must be signed and dated. Once a complaint has been submitted, the MD ADA Compliance Office will review the complaint and contact the complainant for further information, if necessary.
