The Minnesota Instructions — Parenting / Financial Disclosure Statement is a court form used in Minnesota state family court proceedings. This form must be completed and filed with the court by both parties involved in a parenting or financial dispute, such as a divorce, legal separation, or child custody/support case. The form requires both parties to provide financial information, including income, assets, debts, and expenses. It also requires each party to disclose any parenting time agreements that have been reached, as well as any modifications to the existing parenting plan. The form must be signed and notarized in order to be accepted by the court. There are two types of Minnesota Instructions — Parenting / Financial Disclosure Statement. The first is the Standard Form, which is used in most parenting and financial cases. The second is the Simplified Form, which is used in cases involving only one party or when the parties have a very simple agreement regarding parenting time or finances.
