The Minnesota Affidavit of Service By Personal Delivery is a document used to certify that legal documents have been served in a timely manner, in accordance with the law. It is used to provide proof of service of documents such as summonses, complaints, subpoenas, motions, and orders. The affidavit is typically completed by the person who delivered the documents, and affirms that the documents have been delivered in person to the intended recipient. There are two main types of Minnesota Affidavit of Service By Personal Delivery: the Uniform Affidavit of Service, and the Non-Uniform Affidavit of Service. The Uniform Affidavit of Service is used when the documents have been served in accordance with the Minnesota Rules of Civil Procedure. The Non-Uniform Affidavit of Service is used when the documents have been served in a manner other than as outlined in the Minnesota Rules of Civil Procedure. Both the Uniform and Non-Uniform Minnesota Affidavit of Service By Personal Delivery must include the following information: the date and time of service; the name, address, and title of the recipient; the name and address of the person or entity that is the sender of the documents; a description of the documents that were served; the name and address of the person who delivered the documents; and a statement affirming that the documents were received by the recipient. The document must be signed and notarized to be considered valid.
