Minnesota Appendix IV: Affidavit of Emeritus Status is a form that is used by retired Minnesota teachers to request Emeritus status from their school district. This form is used to verify that the teacher has met all the requirements for Emeritus status, including that they have at least 10 years of teaching experience in Minnesota, have retired from teaching, and have completed all necessary paperwork. The affidavit must be signed by the retired teacher and school district representative and submitted to the Minnesota Department of Education for approval. There are two types of Minnesota Appendix IV: Affidavit of Emeritus Status: (1) Regular Status and (2) Active Status. The Regular Status requires that the teacher has retired from teaching for at least one year, while the Active Status requires that the teacher is still actively teaching. Both types of affidavits must be accompanied by a list of the teacher's professional accomplishments, such as awards and honors received.
