The Minnesota Report of Work Ability is a medical form used to assess an individual’s physical and mental abilities to perform work-related tasks. It is commonly used in Social Security Disability Insurance (SDI) and Supplemental Security Income (SSI) disability determinations. The report is completed by a licensed physician to provide objective evidence of the individual’s physical and mental capabilities. It includes sections to assess any physical impairments, mental impairments, and functional abilities, such as standing, walking, sitting, lifting, reaching, and bending. There are two types of Minnesota Report of Work Ability forms: a short form and a long form. The short form is typically used for disability determinations and should be completed by the individual’s treating physician. The long form is used for more detailed and in-depth evaluations, and should be completed by a specialist. Both forms are available online from the Social Security Administration (SSA).
