The Montana Application for Notary Appointment — New Notary and Renewals is a form used by the Montana Secretary of State to appoint a new or renew an existing Notary Public. There are two types of Montana Application for Notary Appointment — New Notary and Renewals: 1) New Notary Application Form and 2) Notary Renewal Application Form. The New Notary Application Form is used to appoint an individual as a Notary Public in the state of Montana. The Notary Renewal Application Form is used to renew an existing Notary Public in the state of Montana. The application must be completed in full and include the following information: name, address, date of birth, employer information, a statement of eligibility, and signatures from two qualified witnesses. Additionally, the application must be accompanied by a $25 filing fee and a $50 Surety Bond. Upon completion and submission of the application, the Montana Secretary of State will review the application and grant the Notary appointment, if approved.
