There are two types of New York SCUDS forms: the Employer Information Sheet and the Partial Payment Plan Application. The Employer Information Sheet provides employers with details on their responsibilities regarding withholding, reporting, and remitting child support payments, as well as information on how to contact the SCU for assistance. The Partial Payment Plan Application is used when an employer is unable to fully pay the withholding amount due on a particular paycheck and must submit a request to the SCU for a payment plan to make up the difference.
