The Oregon Confidential Information Form (CIF) is a document used by the Oregon Department of Human Services (DHS) to protect the privacy of individuals receiving public assistance. It is designed to protect the confidential information of individuals receiving services such as Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANK), and Medicaid. The CIF helps ensure that confidential information remains secure and is only shared with authorized personnel. It contains information such as name, address, date of birth, Social Security number, and other identifying information. There are three types of Oregon Confidential Information Forms (CIF's): CIF-A, CIF-B, and CIF-C. CIF-A is used by DHS to collect information at the initial application stage. CIF-B is used to update information from time to time. CIF-C is used to terminate eligibility and release confidential information.
