Pennsylvania Request For Waiver of Procedures Under Regulation 504 (PW-504) is a form designed to provide a mechanism for financial institutions and other creditors to request a waiver of certain procedures required under Regulation 504 of the Consumer Credit Protection Act. The PW-504 allows creditors to request a waiver of the requirements to provide certain disclosures, obtain written acknowledgement of the debt, obtain a signature of the borrower, or provide a copy of the loan agreement. The PW-504 also allows creditors to request a waiver of the requirements to obtain a credit report and obtain a written statement from the borrower regarding the purpose of the loan. There are three types of PW-504s: Type I, which applies to consumer credit transactions; Type II, which applies to loans secured by real estate; and Type III, which applies to small business loans.
