A South Carolina Physician's Statement is a document that is completed by a licensed physician in the state of South Carolina to certify a patient's medical condition and/or the need for medical care. This document is used for various purposes including Workman's Compensation, Social Security Disability, and other insurance benefits. There are two types of South Carolina Physician's Statement: one for Workman's Compensation and one for Social Security Disability. The Workman's Compensation version is used to certify an employee's medical condition that was caused by a work-related injury or illness. The Social Security Disability version is used to certify a patient's medical condition that prevents them from performing substantial gainful activity, as defined by the Social Security Administration. Both versions must be completed by a licensed physician in South Carolina and include the patient's diagnosis, prognosis, medical treatment and any necessary medical care.
