A Guide for Health Care Agents (also known as a Health Care Proxy) is a document that allows individuals to appoint a trusted person to make health care decisions on their behalf in the event they become unable to make those decisions themselves. The document typically includes instructions and preferences for medical treatments and end-of-life decisions. It is important for people to appoint a health care agent they trust to carry out their wishes. There are two types of Guides for Health Care Agents: 1. Durable Power of Attorney for Healthcare: This document allows an individual to appoint someone else to make healthcare decisions on their behalf in the event they become unable to do so. 2. Advance Directive: This document allows an individual to detail their wishes for medical treatment and end-of-life decisions in the event they become unable to make decisions themselves.
