Form C Civil Appeal Pre-Argument Statement is a document that is filed with the court to provide the court and other parties with a brief summary of the facts and points of law that will be argued in an upcoming appeal. The statement is used to help the court decide whether to grant leave for the appeal and must be filed no later than twenty days after the notice of the appeal is filed. The Form C Civil Appeal Pre-Argument Statement includes the following elements: the name of the appellant and appealed; the case number of the appeal; a brief summary of the facts and points of law that will be argued; the date of the appeal hearing; and a statement of the relief requested. Depending on the court, the Form C may also include additional requirements, such as a certificate of service. There are two types of Form C Civil Appeal Pre-Argument Statements: an Original Form C and an Amended Form C. An Original Form C must be filed at the time of the filing of the notice of the appeal. An Amended Form C is filed when the appellant wants to amend or add to the original statement. Both types of Form C must be filed no later than twenty days after the notice of the appeal is filed.
