The statement for an appeal to the Appellate Division must include the name of the court from which the appeal is being taken, the names of the parties to the appeal, a brief statement of the facts, the legal issues to be decided, and the relief sought. The statement for an appeal to the Supreme Court must also include the factual and legal issues to be decided, the relief sought, and a statement that the appeal is properly before the court.
