The Utah Military Service Declaration is an official form used by members of the Utah National Guard and other branches of the military to officially declare their service in the state of Utah. This form is used to document the service of members who are on active duty, in the Reserve, or in the National Guard. The declaration includes the name of the service member, the service branch, the date of entry into service, the date of separation, the type of service (active duty, Reserve, or National Guard), and any decorations and awards received. There are two types of Utah Military Service Declaration forms: the DD Form 4 and the DD Form 4-1. The DD Form 4 is used to document service in the active duty, Reserve, or National Guard, while the DD Form 4-1 is used to document service in the National Guard only. Both forms are available online and must be signed and notarized before being submitted to the appropriate military branch.
