The Washington Confidential Information Form Addendum is a document used in Washington State to protect the confidentiality of confidential and/or sensitive information. It is also known as the Confidential Information Release Form. This Addendum is required of all state employees and contractors as part of the rules of the Washington State Office of Financial Management. The Addendum is used to protect the confidentiality of certain information from public disclosure. The Washington Confidential Information Form Addendum comes in two types: the Standard Addendum and the Enhanced Addendum. The Standard Addendum is used to protect the confidentiality of confidential or sensitive information that is not covered by the Public Records Act or other applicable laws, such as personal information, trade secrets, or proprietary information. The Enhanced Addendum is used to protect the confidentiality of sensitive information that is protected by the Public Records Act or other applicable laws, such as records of criminal convictions, medical records, or social security numbers. The Washington Confidential Information Form Addendum provides confidentiality agreements to protect the confidentiality of the information being disclosed. It also includes provisions concerning the use, disclosure, and destruction of the information being released. The Addendum must be signed by both parties before any confidential information is released.
