The Washington Firearm Identification Worksheet is a form used to determine the eligibility of a person to possess firearms in the state of Washington. It is commonly referred to as a WA FID. There are four variations of the Washington Firearm Identification Worksheet: Form FID-1 is used for individuals who are not prohibited from possessing firearms, Form FID-2 is used for individuals who are prohibited from possessing firearms, Form FID-3 is used to transfer a firearm from one person to another, and Form FID-4 is used for individuals who are requesting an exemption from the state’s firearm laws.
