Washington Affidavit for Amusement Rides is a document issued by the Washington State Department of Labor & Industries, which certifies that amusement rides and attractions are in compliance with the rules and regulations of the state. This includes verifying that all safety devices are properly installed, inspected, and maintained in order to ensure the safety of those using the rides. The affidavit is issued after an independent safety inspection by a qualified inspector or engineer and must be renewed every year. There are two types of Washington Affidavit for Amusement Rides: the Annual Affidavit and the Initial Affidavit. The Annual Affidavit is required for all amusement rides and attractions operating in the state of Washington that have been in service for at least one year. The Initial Affidavit is required for all new amusement rides and attractions in the state that have not yet been in service for one year. Both affidavits must be completed and submitted to the Department of Labor & Industries before the ride can be opened to the public.
