The application for leave to extend the period of absence of the school administrator or staff of the school district must be received by the board no more than 90 days prior to the date of the student's departure. The application must be signed by the student seeking the leave and, when the student has reached age 19, by the parent or guardian of the student. Any person submitting a request for a leave of absence to extend the period of absence of a teacher or administrator shall submit to the board, in an official paper, a certification of the absence and the date of the departure. The application must be signed by the board authorized to appoint the teacher or administrator in which case the board shall enter the person named as the teacher or administrator in its records. The application and certification must be on a form approved by the governor. The certificate must be clearly legible and contain such information as the governor may require.
