Form 4852 serves as a substitute for Form W-2, Form W-2c, and Form 1099-R, and is completed by taxpayers or their representatives when: Their employer or payer does not give them a Form W-2 or Form 1099-R. An employer or payer has issued an incorrect Form W-2 or Form 1099-R.

If you have Form 4852 (a substitute for Form W-2 or Form 1099-R) you can still e-file the return if you have the employer or payer's EIN and address. Open the return that will be filed electronically.

The Substitute Form 1099-G is mailed in January. This form reports the amount of your state income tax refund paid in the prior year that may be considered part of your Adjusted Gross Income (AGI) if you itemize deductions.
