The Patient Health Questionnaire—9 (PHQ-9) and the Generalized Anxiety Disorder Questionnaire– 7 (GAD-7) are short screening instruments used for detection of depression and anxiety symptoms in various settings, including general and mental health care as well as the general population.

Results. Results showed that the PHQ-9 and the GAD-7 are reliable screening tools for depression and anxiety (Cronbach alpha 0.86 and 0.91, respectively). The one-factor structure of the PHQ-9 and the GAD-7 was confirmed by the Confirmatory Factor Analysis.

However, for the BSL versions the cut off scores for Deaf BSL users have been established as lower following a rigorous process of analysis based on Deaf population responses. The clinical cut-off scores for the PHQ-9 BSL is eight and for the GAD-7 BSL it is six.

Scores of 5, 10, and 15 are taken as the cut-off points for mild, moderate and severe anxiety, respectively. When used as a screening tool, further evaluation is recommended when the score is 10 or greater. Scores represent: 0-5 mild.

The PHQ–9 and CORE–OM perform as well in UK primary care as clinician-rated instruments. GPs may consider using either the PHQ–9 or CORE–OM alongside enhancements of care for depression.

GAD-7 is a seven-item measure of anxiety symptoms. Each item is rated on the same four ordinal responses as the PHQ-9, giving a severity score between 0 and 21. A score above 7 is recommended to identify a likely anxiety disorder. PHQ-9 and GAD-7 form part of the UK Department of Health's National Minimum Data Set.
