Look for an activation code on your After Visit Summary or in an email or text message from your healthcare organization. Follow the instructions provided with your activation code to create your MyChart account.

Here's how to sign the person up for proxy access in Epic: With the child's chart open, select the Epic button > Tools > MyChart Utilities > Proxy Access. Select the child at the bottom of the Patient Lookup window. Anyone who currently has access to the child's information appears in the Proxies section.

For Proxy access to an adult account Select Friends and family access Click on Invite someone. Fill out all fields to send someone an invitation to receive proxy access to your account. Read and agree to the terms and conditions and click on the checkbox and click on send the invites.
