For this article, we will use Continuity of Care Document or CCD. The purpose of a CCD is to improve communication between health care providers during a transition of care – when a patient is being referred to another provider or coming back to their normal provider after a hospital stay, for example.

There are three types of continuity that are present in all healthcare settings: Informational, management and relational.

For example, care managers may arrange for a home nurse to visit or for an aide to help with housecleaning and preparation of meals. They may locate a pharmacy that delivers medications or arrange for transportation to and from the doctor's office.

We identified three types of continuity in every discipline—informational, management, and relational (box). The importance attached to each type differs ing to the providers and the context of care, and each can be viewed from either a person focused or disease focused perspective.

But in some cases, you may be able to keep your doctor, medical group, or hospital for a limited time. This is called "continuity of care." To receive continuity of care, you must call your health plan to ask for continuity of care. Also, your doctor, medical group, or hospital must agree to keep you as a patient.

Continuity of care is concerned with quality of care over time. It is the process by which the patient and his/her physician-led care team are cooperatively involved in ongoing health care management toward the shared goal of high quality, cost-effective medical care.
