This simply means that your previous university should write a letter, confirming that you can understand and speak English fluently. The idea is to ensure you are proficient in use of English language as the language of instruction/teaching in such countries requesting for such letters are often English.

In an English proficiency letter, a professor, teacher, mentor, guidance counselor, employer or other credible authority figure attests to an individual's competency in speaking and writing English.

Acceptable evidence to prove English language proficiency: Results of a recognized English language test (test results are valid for 2 years): Internet-based TOEFL (minimum score of 79) Paper-based TOEFL (minimum score of 550) IELTS (minimum score of 6.5)
