Persons conducting a business or undertaking (PCBUs) must notify their WHS regulator immediately after becoming aware of serious work-related injuries, illnesses and dangerous incidents. These are referred to as 'notifiable incidents'.

Incident. An occurrence, other than an accident, associated with the operation of an aircraft which affects or could affect the safety of operation. Serious incident. An incident involving circumstances indicating that an accident nearly occurred.

A notifiable incident is when: a person dies. a person experiences a serious injury or illness. a potentially dangerous incident occurs.

A serious incident report, is the culmination of an investigation into the background leading up to an injury or death, seeking to identify what happened, how it happened and whether there were contributory causes which can be learned from.

Provide date and time of the incident and when it was first reported, details of witnesses, and a succinct statement describing the events leading to the incident, the details of the incident, the type of work being undertaken, any hazards involved in the work and any personal protective equipment being used.

A serious incident, meanwhile, is any work-related fatality or serious injury or illness and must be reported immediately, whether it involves a worker, contractor or member of the public.

An incident is serious if it causes or could cause harm to an individual, a group, or the public. Serious harm can include physical, psychological, or financial damage. This harm can be physical or psychological and can include death, injury, illness, or harm to mental health.

Serious Reportable Incidents (SRI) are defined as any situation in which the participant experiences a perceived or actual threat to his/her health and welfare or to his/her ability to remain in the community.
