One “6-Clicks” form assesses basic mobility function, and another form assesses daily activity function. Each item is scored from 1 to 4 on the basis of the amount of difficulty a patient has or how much help is needed from another person in completing the task. Lower scores equate to lower levels of function.

For descriptive purposes, we categorized each score into four groups: 6-11, 12-17, 18-23, and 24, where scores below 12 require complete impairment on at least one dimension, a score of 17 or below is equivalent to at least 50% impairment, and a score of 24 implies no impairment [16].

Known as “6-Clicks” due to being composed of six scored patient activity questions, a patient's “6-Clicks” score falls on a 6- to 24-point scale where a score of 6 represents total functional impairment and a score of 24 represents total absence of impairment.

Conclusions: AMPAC scores provide a significant quantitative measure that can be used to predict discharge outcomes of patients prior to admission to an inpatient rehabilitation facility (IRF). Identifying qualified and successful acute inpatient rehabilitation candidates is a comprehensive process.

The original AM-PAC is used with a variety of diagnoses and patient populations to measure basic mobility (with and without a stair-climbing option), daily activity, and applied cognitive functions. The AM-PAC measures aspects such as difficulty, assistance, and limitations in activities of daily living.
