- Send the IEP Invitation and Meeting Response Form to both the parent and the Local Education Agency (LEA).
- Send copies of IEP2 and the IEP Amendment Phone Letter (IEP3) to the parent and the LEA.
- Place the original copies of IEP2 and IEP3 in the student’s records, attached to the current IEP.
- If convening the IEP team, complete the Amendment Form (IEP2) with input from all attendees.
- Provide both the parent and the LEA copies of the completed IEP1, MRF, and IEP2 if the parent attended the meeting.
- If the parent did not attend, mail them the completed IEP2 along with a letter (IEP4) to obtain their consent.
- Once consent is received, share copies of the completed IEP1, MRF, IEP4, and IEP2 with the parent and LEA.
- Ensure all original documents including IEP1, MRF, IEP2, and IEP4 are stored in the student’s file.
- If the IEP team denies any requests made by the parent, complete and provide a Prior Written Notice form to both the parent and the LEA.
