Medically reviewed by Neil Chatterjee, MD. Random pill counts are a common protocol that doctors use to ensure that the patient is using the correct drug at the correct intervals. If your doctor is conducting a pill count, it usually doesn't mean that the doctor has any reason to distrust you.

Ask the patient to bring in the medications at each and every visit, and then count out the medications to confirm that they are on track with the prescribed schedule. Another strategy is to have the patient come back in between visits to conduct a random pill count.

If you don't respond to a request or never show up, your provider may refuse future prescriptions or even release you from their practice entirely! These policies vary widely from provider to provider, so it is always best to fully read any agreement you have signed, or just ask the office for more clarification.

Failure to appear for a random pill count or have accurate counts will result in immediate discharge from the practice. This policy exists so the practice can attempt to protect against misuse, abuse or diversion of the medication and protect the practice from unwanted scrutiny by supervising governing organizations.

Adherence measured by pill count was calculated as the percentage of the number of prescribed pills corrected for the number of returned pills divided by the period (in days) multiplied by 100%. For both methods, an adherence level of at least 90% was defined as acceptable.
