Required for all research involving human participants not at a Regulated Research Institution. If at a Regulated Research Institution, use institutional approval forms for documentation of prior review and approval. (IRB approval required before recruitment or data collection.)

There are several forms of research: scientific, humanities, artistic, economic, social, business, marketing, practitioner research, life, technological, etc. The scientific study of research practices is known as meta-research.

Must be completed before experimentation. Student's Name(s) Title of Project. To be completed by the Student Researcher(s) in collaboration with Designated Supervisor/Qualified.

Risk Assessment Form (3) (pdf) May be required for projects involving Hazardous Chemicals, Materials or Devices or Potentially Hazardous Biological Agents; recommended for all projects.

Continuation Projects Form (7) Explain as completely as possible how the project is different from previous experimentation because ONLY a new and different research project is allowed. The date signed is the date the student researcher is certifying that this information is correct.
