PRINCIPAL PURPOSE(S): To obtain medical data for determination of medical fitness for enlistment, induction, appointment and retention for applicants and members of the Armed Forces. The information will also be used for medical boards and separation of Service members from the Armed Forces.

The information collected on this form is used to assist DoD physicians in making determinations as to acceptability of applicants for military service and verifies disqualifying medical condition(s) noted on the prescreening from (DD 2807-2)/.

Your MEPS physical is good for two years. If you do the Delayed Enlistment Program (DEP), you will need to undergo a quick height and weight (BMI) check before shipping out to Basic Training, but this is not as in-depth of an exam as the original physical.

Separation / Retirement physical Patient must complete DD 2808 "Report of Medical Examination". Patient must have results of a current audiometric test documented on page 2 Box 71a-73 of DD 2808.

A complete physical examination is required every five years. Each five-year periodic physical exam is valid through the end of the month from the date signed by the examiner.

A DD 2808 Form is used by the US Department of Defense when an individual wants to join one of the branches of the US military. Known as the Report of Medical Examination, the document is completed when a recruit enters into the Military Entrance Processing Station (MEPS).
