Get Seattle University Immunization Form

For privacy and protection of your personal health information we only accept records uploaded in your patient portal. You will need to log in the patient portal and upload your immunization records in the medical clearance section.

To submit your immunization records, select the Update button next to Immunization Record in the Medical Clearances category. Select the Upload button, then upload and save your immunization documents.

Please submit all immunization related documents to Albany State University; Student Health Services 504 College Drive; Billy C Black Building Rm 138; Albany, GA 31705 or FAX to 229-500-4918.

Submit any immunization records to either immunizations@gsu.edu with panther ID# written on the document or to the student portal at health.gsu.edu. Other Holds: If your registration hold is not immunizations related, please contact GSU Admissions office at 404-413-2000 for further help.

Seattle University requires all students enrolled in undergraduate courses to provide documentation that they have had two doses of Measles, Mumps and Rubella (MMR).

WSU system-wide will require proof of the COVID‑19 vaccination for the 2022–2023 academic year for all students engaging in activities at a WSU campus or location.

Immunization Records can release your immunization records in one of the following ways: You can pick up your records in person at Immunization Records, Room 307, North Building (a valid photo ID is required) Or you can email wellness@hunter.cuny.edu, or mail the Consent to Release Immunization Form.
