If you already have a forwarding order in the system and need to change the order, contact your local post office. They will complete a PS Form 3546, Official Change/Correction to Mail Forwarding Change of Address Order.

PS Form 3665 is known as the Certificate of Mailing – Firm and is used for multiple pieces in a single order or mailing. The form allows mailers to list multiple items by line and ensure that all items are accounted for with a proper acceptance for the mailing date. The form then becomes the receipt, as with all forms.

Plant-verified drop shipment (PVDS) enables origin verification and postage payment for shipments that a mailer transports from the mailer's plant to destination Post Offices™ where the prepaid and pre-verified shipments are accepted by the Postal Service™ as mail.

Address Service Requested provides two services: (1) the mail piece is forwarded to the new recipient address if possible and (2) the sender receives back a USPS®-sent postcard called a Form 3547 (Notice to Mailer of Correction in Address) which details either the new address or reason for non-delivery.

PS Form 3602-EZ is a simplified postage statement designed specifically for small business mailers who are mailing USPS Marketing Mail letters, and flats. To download a print-only version of PS Form 3602-EZ, click on ”Postage Statements” in the left frame of Postal Explorer.

Postal Service forms may be preceded by “PS Form,” which simply stands for “Postal Service Form.” For example: “3541,” “Form 3541,” and “PS Form 3541” all refer to the Periodicals postage statement.
